Cut a tip on your tube that is *just* smaller than the crack that needs to be filled. Angle your tube against the crack, and squeeze an even amount into the crack. Now, use your finger (gloved, if you want, because this stuff is pretty sticky) and smooth the line down, pressing the caulk to fill the entire crack.
Absolutely remove the old caulk. The old stuff is obviously breaking apart, and will continue to do so. If you put new caulk on top, it's going to fail as the old caulk continues to break apart and no longer adhere to the surface below.

Your best solution is a heat gun(NOT A TORCH), moving slowly down the caulk line to soften it, and scrape as you go with a putty knife. After doing this you can go at whatever remains with a nylon brush and caulk solvent. Isopropyl alcohol also can work instead of the solvent.
